This survey is to help the NHS and other health and care organisations in south east London better understand your views about your health, wellbeing and what is important to you right now.
Your answers will help us learn more about your health, how healthy you are feeling and any support you might need to be as healthy as possible.
If you live in Bexley, Bromley, Greenwich, Lambeth, Lewisham your local NHS South East London would like to hear from you. We will use what you tell us in this survey to develop and inform our work now and in the future as we help people to be as healthy and well as possible and get the care and support they need.
