Have you visited hospitals or community services recently?

If you have attended an appointment at University Hospital Lewisham, Queen Elizabeth Hospital in Woolwich or used community services in Lewisham in the last 12 months Lewisham and Greenwich NHS Trust want to hear from you.

They want to hear about your experiences so they can create a better and more inclusive healthcare setting for LGBT+ people. They  welcome feedback from all their patients and service users, you do not have to identify as LGBT+ to share your thoughts.

This feedback will help to demonstrate the progress that is being made within LGBT+ inclusion and help guide further improvements.

It only takes a couple of minutes to complete the survey and all responses are anonymous.
